Example time card online?/Using Hy-Tek for small masters meets

Does anyone know where one could find a sample time card that is viewable online? Either a scan of a time card or a pdf file or Word file or anything? I've filled out so many I should be able to remember all the various boxes, but I can't.

Although I sent you a link to a sample of the timer's cards we use in our meet, it is not quite what you are looking for. Sorry.
Look in the LMSC handbook records chapter (Administration section of the USMS web site) for a sample "relay entry card" - it may have a lot of the information you are looking for and could be modified for individual events.
