For dryland sports or work efforts, exhale on muscular exertion is the correct method. But for racing starts in swimming, I prefer to inhale during flight to get a big gulp of air before I hit the water.

I've noticed that in order to bend over and grab (track start here), I have to exhale some, so, when I launch, I inhale.
I don't know if that's a flexibility issue, or maybe just plain getting fat.
I have the same problem and exhale on the way down, and inhale mid-start.
