We've run 2/lane in the past a few times for the 400, 800 and 1500 freestyles. First heat starts and when they get down around the 35-45 meter mark of their first lap, second heat goes off. This year the number of entries were of manageable size so all but one heat swam 1/lane.
It's actually fairly easy and there have only been a couple of minor challenges with it. I've yet to see a swimmer in the second heat false start.
A couple of things you can't do is get "official" split times from the pads as they are inoperative. I also seem to recall FINA won't allow any world records to be recognized competing in this format.
