In another thread someone was mentioning getting out of practice early because they weren't doing well. When, or why, have you gotten out early?
I'll start:
(1) Kicked in the face (really hard, big owies!)
(2) Kicked in the shoulder (by big guy 6'4" 200#s, I call him husband!)
(3) Decided to swim with aforementioned husband immediately after putting to sleep 14 year old black Lab. We thought it would take our minds off of MacGuire. All I discovered was that I can't swim and cry at the same time.
(4) Bad back- more owies. Good chiropractor- all better.
And I SHOULD have got out (Jan 2001) when:
I swam the whole workout, with good effort, and was shivering cold. I told my husband that I was really cold and he replied, "Maybe you should swim some more (tonight)." I said, "Nah, we're done, I'm getting out." That night I went to the ER and was diagnosed with viral meningitis! Nope, he still hasn't lived THAT one down :p

Cramps, cramps, cramps! I also got out once because I was depressed and in a bad mood. I once had a headache and didn't like the person leading the lane, so I got out. The worst one was when I injured my back going off the starting blocks (Feb. 2006) and couldn't move my right arm! (scccaarrrryyyyy......)
I've WANTED to get out a lot (but stuck it out - always glad I did), especially during a: 1) really painful set, 2) I'm stuck with people way too fast for me
I was dragging and needed a day off--and we had 18 people including me in a 50 meter lane--so all I was doing was taking up space to no real purpose since I wasn't doing anything worthwhile and we badly needed fewer people in the lane....

yikes! I have heard that is not the best of situations... Y'all probably weren't doing anything longer than a 50 leaving 3 sec apart were you? I think the worst I have seen it in LCM (aside from warmup at savannah nationals) was ~10 people. we split our lane in two and had half start at one end for the whole practice and the other half at the other end. it helped make it feel less crowded.
Back in summer swimming
I got out when the coach, and at the time my boss got really pissed at me. He ever so politely asked me to leave and made me pay for it that evening at pm practice. I planned to ditch pm practice but since he was at work he saw to it that I made it there.
Other than that I just leave early to go to work on the off chance I get up for an AM practice.
Got Boost
I actually got out early today. The pool heater broke over the weekend, and the water was really cold (it's usually 82-86, today it was 76), and I was shivering during the main set. The chlorine levels were also really off. I'm swimming at swim club nationals up at UVA this weekend, so I decided it was better to get out than to wear myself out trying to stay warm.
