How much do you typically vary your workouts and their intensity?
For the most part I'm a short to mid-distance freestyler, and do mainly various sets of 50s, 100s, and 200s. I think I tend to get into phases where I stay in the 70-85% effort level and after awhile start to feel stale. Maybe once every two weeks or so I'll be very tired at the start of the workout, planning to do more mediocre 100s. So I decide to change the workout and do some fast 25s. It usually adds excitement and motivation to put forth more effort. How often should you incorporate fast swimming, say 90% effort and above? I believe part of the problem is the feeling that I need to complete X number of yards in a workout and if I sprint there isn't enough time to get X number of yards in due to the shorter distances and more rest.

I enjoyed reading that second link.
Those were some horrible sets. That 5,000 for time by Michael Phelps is astounding.
The age group team I work out with has varied days for differnt things. Mon/Tue usually normal but hard practice, Wed is either distance or sprint stroke, Thur is middle distance but very intense. Fri is recovery day usually lots of kicking and Sat/Sun are painfull 7,000-8,000 yard practices. Here is practice I did yesterday evening with the kiddos.
