I found this from Munich 2000:
A swimmer may enter a maximum of 5 (five) individual events and 2 (two) relays, but not more than two individual events per day.
I looked at the entry form for Christchurch (NZL) 2002 and found the same limitations stated on the form.
I could not find anything from Riccione 2004 but would venture a guess it was the same.
I'd also guess that it'll be 5 max for the meet and no more than 2 per any given day also at Stanford.
