Thanks for the advice.  I am going to see the orthotic guy tomorrow.  Did you ever use any special insoles/pads in your shoes in addition to your orthotics? I have worn custom-made orthotics for 29 years.  A couple of years ago I started having pain in the ball of my foot.  It turned out that my orthotics had &amp;quot;collapsed&amp;quot; and were no longer providing enough support.  (They were plastic and were about eight years old.)  I got a new pair made and that solved the problem.
