I used to wear two caps with the goggles in between the two; however, I found it to be too hot and constricting.  I switched last year to competing with just one silicone cap with my goggles under it.  It seems to work just as well, because I have never lost my goggles in a race.  Putting Vaseline around my eyes helps the goggles fit better, and keeping the strap around the crown of my head helps as well. If your eyes are forward, the goggles will fill with water, or come off. Instead, tuck your head a little, eyes down, and the goggles will stay on.
