It seems to me that you are very flat when you swim. This makes you swim mainly with your arms rather that your upper body. In my opinion you need to rotate from side to side more. 
 
The longer you swam the wider your arm pull was underwater. It is difficult to tell exactly from that shot but it seemed that your pull was getting outside the line of your body the longer you went.
